Four separate symptoms with a short list of causes between them. Work down the one that matches.
Confirm a profile has actually been loaded: open GUIConfigTool to check, or re-load it. Note that an off LED is normal — it means no button was held at boot, so the default profile is running. It does not mean a profile is missing.
The cabinet connects to one host at a time: if another Quest or PC is still connected to it, switch that host's Bluetooth off. This can also happen right after a profile change: profiles of the same kind (keyboard or gamepad) use the same Bluetooth address, so a Quest paired with XA reconnects to the cabinet when you load VirtuaTilt-PC. With firmware older than v8.0.19 the cabinet could also stay invisible after you removed it from a host or turned Bluetooth off and on, until it was switched off and on: update both chips (Appendix A).
A previous profile is still loaded, or you forgot to plug in over USB and click Program Pico. Re-load the correct profile; the pairing name comes from the profile, not the hardware. With firmware v8.0.18 and later the new name appears about 15 seconds after Program Pico (see Loading a Profile).
Forget the pairing on the host, toggle Bluetooth off and on, and pair again — the host's HID descriptor cache may be stale from a previous session. On Windows you can also restart the Bluetooth services from an Administrator PowerShell:

With firmware older than v8.0.19 the buttons, or the nudge and plunger, could also stop working over Bluetooth after the cabinet restarted (power-on, profile change, update), until it was paired again. Fixed in v8.0.19: update both chips (Appendix A); existing pairings are put right when the cabinet starts.
Almost always a stale pairing on the Quest side. Forget the VirtuaTilt pairing on the Quest, toggle Bluetooth off and back on, and pair fresh. Confirm your firmware is current while you are there: GUIConfigTool → Overview should show Firmware status: up to date for the Pico and the ESP32 module.
If a stubborn pairing will not clear, reset the Quest's Bluetooth stack — we suggest the third-party Quest Games Optimizer (QGO) tool for this.
For a guaranteed-stable connection you can also sidestep Bluetooth altogether and switch to USB (wired) mode: load the matching USB profile and connect the cabinet to the Quest through a USB-A → USB-C OTG adapter.
